Ages 0 to 19 years old $55/mo
We care for children of all ages, helping you form healthy habits with your children and adolescents. Wellness exams, sports physicals, routine sick and acute care. $70/mo per child if no enrolled parent(s). Membership fees per family will be capped not to exceed $325/mo for 2 adults and all household children up to 4 children (additional $25/mo for each additional child thereafter).

Ages 20 to 39 years old $79/mo
At this age, life-long habits become solidified amidst many other ongoing life changes. We understand you may need to access us for routine or sick care while away at school, traveling for work, or during transitions between jobs. Direct care membership makes it seamless.

Ages 40 to 59 years old $95/mo
Aaah, midlife. Now more than ever, care focus shifts to preventing and screening for chronic diseases and optimizing physical and mental health and wellness. Accessing healthcare needs in a direct care model provides flexibility, no long wait times, and extended visits when needed. Whether it's travel, career changes, or the juggle of the sandwich generation - your time and health are valuable and worth investing in.

Ages 60+ years old $110/mo
Access to the time you need with a physician that knows you, your care goals and health needs. Continued focus on prevention of disease, and active chronic care management to keep you living life with intention and engagement. Care coordination with specialists, family members and care givers. Working with you so that your loved ones know your wishes. Flexible care delivery even if you're unable to leave your home (*home visits may carry additional fee).

Prenatal Care $250
Includes all pre-conception counseling and pregnancy care up to 28 weeks gestation, with transfer to insurance based Ob/Gyn or Family Physician for 3rd trimester and delivery care. Should pregnancy loss occur, or transfer of care be indicated sooner than 28 weeks due to medical complications, this is prorated to $50 per each completed 4 weeks of prenatal care at rootsMD. *Patient is responsible for all additional fees associated with prenatal laboratory and ultrasound testing.

Initial Enrollment $99/pt or $175/family
Family rate applies for up to 2 household adults and dependents; must enroll within 4 weeks of initial member. Re-enrollment fees apply for members that discontinue membership and wish to rejoin the practice at a later date.
At rootsMD we can take care of 80 - 90% of what you may need. However, we recommend that you have health insurance for complex specialty, unexpected or catastrophic needs.
Some patients continue a commercial insurance plan in addition to a DPC membership for the benefits of improved relationship with and access to their own physician, and DPC access to lab and medication savings. Self-funded employers or small businesses may opt to offer a wrap around plan with DPC for their employees. Some will utilize a major medical plan with a high deductible and health savings account (HD/HSA). Other patients will use health cost sharing plans – some of these plans offer discounts to DPC patients knowing they are less likely access unnecessary and costlier care away from their PCP office.
